Attendant Care Assessment (Form 1)

Our trained, CAT-Certified occupational therapists provide In-Home Attendant Care Needs Assessments.  We provide an in-depth evaluation of the client’s ability to perform their basic and essential personal care activities, and assess their safety within the home environment as a result of their physical, cognitive, or psychological injuries. We provide a detailed report that outlines our findings,  recommendations for assistive devices required and the costs associated with providing these assistive devices as outlined in the Assessment of Attendant Care Needs (Form 1)

Occupational Therapy Assessment and Treatment (CAT-Certified)

Our occupational therapists are skilled in providing services for individuals of all ages across a wide range of conditions, including orthopedic injuries, chronic pain, acquired brain injury and concussion, mental health conditions, chronic illness, amputation and spinal cord injuries.

Our Occupational Therapists work with a Multidisciplinary team (Psychiatrists, Neuropsychologists, Orthopaedic Surgeons, Neurologists, Physiatrists, etc.). They have extensive knowledge of the SABS and various community resources to ensure our clients are provided with collaborative care that maximizes their recovery.

During an assessment, our occupational therapist typically will: 

  • Gain an understanding of the client’s pre-injury status:  How did the client engage in activities before their function decreased?
  • Standardized testing and observation of the client doing functional tasks in their real environment(s). What are the physical, cognitive and emotional barriers affecting the client’s ability to engage in daily activities?  What physical, emotional and social supports does the client have in the home, at work or in their community?
  • Develop a collaborative plan for treatment.  What specific skills or supports require improvement?  Are any additional equipment or aids needed? Is education or ongoing treatment necessary?  How does the treatment plan reflect the client’s specific needs and goals as current best practice?
  • Write a report for the insurer and other health providers.  What information would other parties benefit from knowing about the client’s situation?  What objective, measurable goals will help a client to their pre-injury function?

Case Management

Our case managers provide you with one point of contact to ensure continuity of the rehabilitation process, from early intervention until clients reach their optimal recovery in activities of daily living, social, leisure and return to work.

We provide case management services to children and adults for non-Catastrophic cases.  Our experienced case managers have an in-depth understanding of the Statutory Accident Benefits Schedule (SABS) and are registered providers on Health Claims for Auto Insurance (HCAI).

Our Occupational Therapist case managers are experienced with:

  • Coordinating treatment and services which facilitate recovery
  • Establishing an appropriate rehabilitation and medical team based on the client’s needs and making the necessary referrals
  • Communicating with Adjusters and Lawyers while advocating for the client’s individual needs
  • Communicating with hospital personal, medical specialists, therapeutic health care professionals and school personnel to coordinate care and ensure your client’s needs are met
  • Coordinating referrals to medical specialists available in our network
  • Meeting and communicating with clients and their families to obtain a thorough understanding of their needs;
  • Assisting clients to access various supports, resources and services including personal care, support groups, home care, housekeeping and/or transportation to facilitate a smooth reintegration to their home, school and community life
  • Reducing client stress by assisting with paperwork, completing forms, and/or finding support for caregivers
  • Helping clients work towards returning to school or work and reintegrating into the community

Physiotherapy Assessment and Treatment

Our physiotherapists can provide a comprehensive assessment of an individual’s physical and functional abilities. We can identify functional limitations and develop a suitable and effective treatment program aimed at restoring function according to best practice standards. We can also review a current treatment program and, if necessary, make recommendations aimed at improving outcomes according to best practice standards.

Concussion Assessment & Rehabilitation

Our clinicians (physicians and physiotherapists) have advanced training, knowledge and years of experience assessing and treating clients who have suffered a concussion or mild traumatic brain injury.

Chiropractic Assessment and Treatment

Performed by a certified Doctor of Chiropractic, these evaluations identify an individual’s current diagnosis, prognosis, and treatment requirements from a Chiropractic point of view. Chiropractic Evaluations may be focused on developing a detailed treatment plan to address the client’s needs, or it can be used  to review another practitioner’s treatment program and, if necessary, make suggestions for improved outcomes. In either scenario, outcomes are based on current chiropractic best practice standards.

As a Registered Physiotherapist, Michelle has been dedicated to helping individuals improve their health and well being for over 25 years. She has treated clients of all ages and activity levels, and has successfully helped them recover from a wide range of orthopedic and sport injuries. 

She works closely with a multidisciplinary team of physicians, surgeons and allied health professionals to provide integrated care that treats the root of the problem. Her extensive clinical training and experience has allowed her to develop an in-depth understanding of the varying and complex needs of clients She uses this expertise to ensure that clients at PhysioPlus Health Group receive the care they need to achieve their goals.  

As founder and director of PhysioPlus Health Group, Michelle has gathered and mentored a multidisciplinary team of highly skilled practitioners, as well as a supportive administrative team who truly want to help people. Together, she and her team are passionate about providing an outstanding patient experience and delivering exceptional care that helps each patient reach their goals. 

Michelle has been the driving force behind the development and delivery of innovative treatment programs for Knee Arthritis, ACL Injury and Injured Runners, and Motor Vehicle Accident Injuries, as well as an Urgent Sports Injury and Fracture Clinic, Pre/Post Natal Programs and a Therapeutic Pilates and Yoga Program. 

Michelle has a Bachelor of Science in Physiotherapy from the University of Toronto (1992) and a Doctorate in Osteopathic Manual Practice from the Canadian College of Osteopathy (2004).

Dr. Nguyen completed his orthopaedic surgical training at one of the most respected programs in Canada, at the University of Western Ontario. He then pursued further subspecialty training in Orthopaedic Sports Medicine with a Post-Doctoral Fellowship at the prestigious Columbia University / New York Orthopaedic Hospital. After practicing a few years in North Carolina, Dr. Nguyen has recently relocated to Toronto where he continues in his field of sub-specialization: Sports Medicine / Arthroscopic Shoulder, Elbow & Knee Surgery.

Dr. Nguyen has a busy sports medicine practice but also sees active individuals with osteoarthritis, as well as tendon, ligament and cartilage injuries. He enjoys working collaboratively with other health practitioners, treating clients of all ages from recreational and developing athletes to elite athletes, as well as active individuals and those wishing to become more active.

Research is an integral part of Dr. Nguyen’s practice. He reviews scientific articles for many academic journals and has presented his research at multiple national and international meetings.

Dr. Nguyen has earned numerous research and teaching awards in recognition for his achievements in the advancement of orthopaedic knowledge and computer assisted surgery, as well as for outstanding leadership and dedication to medical education. He is actively involved in the training of new primary care physicians, sports medicine physicians and orthopaedic residents from the University of Toronto and McMaster University. In 2007, Dr. Nguyen was recognized as an Emerging Leader of the American Orthopaedic Association by Dr. Louis U. Bigliani.

Jennifer McConvey is a Registered Physiotherapist and certified Vestibular Physiotherapist with over 12 years working in orthopedics as well as concussion and vestibular rehabilitation.

After graduating summa cum laude from the University of South Carolina, she went on to receive a Doctorate of Physical Therapy from Northwestern University’s Feinberg School of Medicine.

Jennifer has received training and holds certifications in vestibular and concussion rehabilitation, as well as various manual therapy techniques. Her expertise in orthopedics, concussion and vestibular rehabilitation has given her the distinguished opportunity to work at some of today’s leading medical institutes, such as Massachusetts General Hospital in Boston and Spaulding Rehabilitation.

Jennifer has held licensure and successfully treated clients in Ontario, Illinois, Massachusetts, Georgia, South Carolina and California. She is registered with the College of Physiotherapist of Ontario and in Massachusetts under the Board of Allied Health Professions and also a member of the Canadian Physiotherapy Association (CPA).
